Smile. A mask of happiness hiding the pain, guilt, turmoil and suffering on the face beneath it. But in the case of Monkey D. Luffy, he isn't smiling because he's happy, nor because he was hiding his pain. It's a smile that conveys no happiness nor sadness. Only millenia of madness. Insanity. Bloodlust. The revenge of the Sun God destroyed the very fabric of reality, space, and time itself, in just a single smile. This had major consequences, but Luffy is more than happy to face it. After all, it's his chance to regain his true smile. Then again, it will never be the same - not when his smile no longer held the same innocence. 

But coming back, he realized that he had to protect HER smile. He can't afford to make her fall down the dark path like he did. She is his sunshine. His anchor to sanity. Luffy will stop at nothing to keep that smile innocent, like he himself had so long ago.
