After Ian's release, Monica manages to talk her ideas into his head. 
That the people around him may have good intentions, as in helping him, but will always feel upset about his state. That he should be around people who accept him for "who he is." 
Ian had seen his mother's struggles all first hand and god knows that he didn't want to take his medication so, it was Monica's words that he saw fit. She must've been right. After all, she was the only one in his life who could truly understand what he was going through. 
He'd just have to find people who would accept him for his "fucked up" self.

Ian breaks up with Mickey.

Mickey, of course, refuses to admit how much this hurts him and resorts to drinking while Ian's adjusting to his meds, as he's practically been forced to go back on them by Fiona.
