25 years after the Locust War ended and the planet of Sera was finally in peace, with humanity beginning to rebuild its civilization. Still, problems arose within the newly-reformed Coalition of Ordered Governments as its government, now under First Minister Mina Jinn, returned to its authoritarian approach and began silencing any peaceful protests against it, creating a sense of unease amongst its populace and its armed forces. This unease then rose sharply when a string of missing persons case appeared within the COG Settlements and Outsiders Settlements. 

Except for one: Phoenix or previously known as Settlement 2, now under the rule of Colonel Tommy Fenix, the oldest son of war heroes Marcus Fenix and Anya Stroud, alongside his organization, the COG Reformist Union or the Reformers, and his military unit, Phoenix Company. He and his organizations were very popular amongst the general populace, the disillusioned Gears, and the Outsiders Movement for the reforms they made that were beneficial. Seeing these mounting missing person cases as well as Mina Jinn's accusation towards the Outsiders for these cases, Tommy initiated a widespread investigation and sent search parties in COG Settlements and Outsiders Settlements.

However, when he personally led a search party to Fort Umson, they saw it under attack by strange creatures and thus, defended the settlements from it. Now, joining forces with JD Fenix, Del Walker, Kait Diaz, his parents and their old teammates, they must once again take up arms to fight against an old enemy, now evolved and more dangerous than ever before, as well as meeting extraordinary allies beyond their world.
