After Lucifer was set free a few months after they met Crowley, Dean was killed, Angel's were turned into Slaves, Humans were forced into hiding, and Demons ruled the world with their leader Lucifer, and the Croatian virus plagued the world. Sam, well, he was turned into Lucifer's slave after being affected by the Croatoan virus. Seven years later Dean returned, but he wasn't the same.
