Muzan was known as a selfish and manipulative man, but really, he was a sheep in wolf's clothing. Muzan was forced to try to push down his trauma, and start his life as the demon king, but the days of torture still haunted him. It's like no matter how hard he tries to forget the past, the past always finds its way back inside muzans little head. Day after day, muzan tries to cope with his trauma by himself, but it only worsens. Soon enough muzan starts to have nightmares, and hallucinations, it's like the universe wanted him to suffer the horrid memories of his past. 

Kokushibo was upper one, known to be the closest demon to muzan and the most loyal, he started noticing something strange about muzan, muzan would space out or when kokushibo tapped him he would jump although he knew kokushibo was there, a lot of weird stuff, and most of the things concerned kokushibo, and it became worrisome, so kokushibo tries to help muzan, hoping for muzan to open up and tell him why he's like this.
