Samuel "Sam" Carter is a quote-on-quote patient at Arkham Asylum. He's been there for over a year, undercover while getting intel on someone that has been captured and being tortured. 

He's there on a mission, sent in by his partner; Ghost-Maker. He is to Ghost-Maker how Robin is to Batman. He goes by the codename; Wraith. Sam is here to rescue Robin from the asylum. 

And he's going to do whatever it takes to get him out. He's even willing to kill everyone that stands in his way.
