Imran  had to witness his mother being on organ support his whole life. And he had to witness her heart being donated to his cousin once his mother died.

His cousin, Zahra, was suffering from a severe heart disease and needed transplant So when his mother died, her heart  was transferred into Zahra's body.

Years later, khalil decided to join them in a marriage because of the connection Zahra has with them all.  While khalil sees his wife in Zahra,    Imran sees Zahra as the complete opposite of that.

To him, she snatched his mother's heart, she could never replace his mother.
