Johnny "Soap" Mactavish was used to being threatened by his fellow packmates, Task Force 141, especially by Ghost. However, an assignment to both Alpha and Omega sent the pair to an undisclosed territory out in the Pacific named innocuously The Regions. Reports came from a criminal ring, run by the infamous Hounds, and led by an Alpha. However, Soap and Ghost find themselves well over their heads when they realize the Alpha is also a combat veteran, and well-versed in psychological warfare.

The pair find themselves caught in the crossfire. They can play along with the delusional assumptions of the Hounds, or play martyrs and uncover the truth about the Hounds, and the Alpha who cares for them. Soap and Ghost have to think fast on their feet, or get caught up in the pack of the Hounds...
