The idiot is Shinya because this is Guren's POV but in fact Guren's the idiot. Shinya got injured and turns out he was feeling ill the whole time, Guren the most worried "Friend" was the one who worries the most.
Or Shinya falls sick and Guren takes care of him, Eventual confession.

I decided to start rewriting it, but I've been procrastinating since its SIXTY THREE PAGES LONG. I kept telling myself "well its readable" no it really isn't. The dialogue is corny and unrealistic and I was making up shit as I went. At least now I have a clue of what the general direction of the story was. And I'm going to make cover art as well, because AI covers look dumb and wrong.
