There are the two pairs of twins. Narda Custodio who is the very kind and helpful EMT Officer and the department head. Narda's twin named Sylvia. Also known as Heart. Sylvia has been very bad girl ever since her and Narda's mother died. Sylvia blames Regina Vanguardia the richest lawyer in town. Regina Vanguardia is Narda's girlfriend which disgusts Sylvia. Regina and Sylvia became rivals ever since Narda's sister started to blame her for the mysterious death of their mother. And Regina also has the twin sister named Iris who is now the big time chef in Bangkok, Thailand.

In short, Narda and Iris are the kinder person. While Regina and Sylvia are nonchalant, toxic.

Sylvia may have become evil, especially when her twin sister Narda is now in a relationship with Regina. But as three of them will happen to cross paths with Regina's twin named Iris, Sylvia will suddenly start to feel something she never felt or forgotten to have felt before.
