They're orphans who all share a room due to having medical health issues. Leo has OCD, Raph is bipolar, Donnie is autistic, Mikey has ADHD. They all have PTSD due to how they lost their parents. Splinter adopts them.

After a trip to the junkyard, Donnie and Mikey unknowingly bring home a small, wooden box. Accidentally opened, the box unleashes a thousand-year-old Demon whose only desire is to feed on the fear of others.
