Sirius had faced a lot, the traumas of growing up in the Black household were not easy ones to endure, but Regulus was a Black. As was Bellatrix. And Narcissa. And Andromeda. A fanfic diving into what the Black children faced in their childhoods.
The Black children were tainted with things normal children shouldn't have to endure.
