Gameplay divergence because the Reaper is an ACTUAL THREAT (like in Striker's, god the Reaper is tough in Striker's) Shumako, Other changes to the lore, like Haru works with Ren in Leblanc, Sojiro adopted Ren, Persona's can communicate with their User's in the real world, and a few other little tweaks and stuff.
