Todd Anderson is drowning. Not literally, but he is drowning. Drowning in expectations he can never meet. Drowning in his brother's shadow even while that brother is trying to keep him from sinking past saving. Neil Perry is also, not literally, drowning in the things expected of him. Med school and a doctor, when all he wants to do is act and his friends are desprately trying to keep him afloat. They're both drowning, the same body of water but world's apart. And when they meet someday in the future, it'll all be okay.
