When Sam Winchester was 16, he slept with some girl and he kept contact but a few months later she dropped some baby on the motel's doorstep, all it said was her name was Elle. A few years later Sam is having some job interviews, living in an apartment with his Girlfriend Jess and his Daughter Elle, until one day Dean shows up.
