This alternative ending to Mafia: The Old Country rewrites only the final stretch of the story, staying faithful to the game's tone, characters, and events while changing the fate of Enzo and Isabella.

After trying to flee the Torrisi villa, Enzo and Isabella are ambushed at the train station and torn apart. From there, the ending splits in two: Enzo is dragged back into Don Torrisi's world and forced into a final chain of brutal confrontations, while Isabella fights to escape the nightmare waiting for her. With Cesare, Tino, and Don Torrisi standing between them and freedom, both must survive their own path through blood, fire, and loss.

This is a darker, more personal reimagining of the ending, where Enzo stays true to who he is, Isabella is given her own fight, and the two of them struggle not just to live, but to finally leave Sicily together.
