Dr. Eleanor "Nell" Carver is one of the senior residents in Pittsburgh's busiest ER . Relentlessly capable, and far better at saving other people than taking care of herself. Between endless night shifts, traumatic cases, and the quiet exhaustion of her own treatment, Nell is trying to hold together a life that never seems to slow down long enough for her to breathe.

The one constant in the chaos is Dr. Jack Abbot - the attending physician who knows exactly when to push, when to stay quiet, and when to simply stand beside her. Their relationship is steady, private in all the ways that matter, and built in the spaces between ambulance sirens.

But while Nell spends her shifts fighting for strangers, the people closest to her are struggling too. Her younger sister, Lois - the hospital's head chef - is trying to keep everyone fed while pretending she doesn't need looking after herself. And Nell's best friend Frank Langdon, freshly separated and emotionally unravelling beneath it all, finds himself unexpectedly drawn toward Lois in the middle of the hospital chaos.
