Hermione had just finished the war and found out that her muggle parents weren't her real ones.  
Her real parents were Daneel and Dean Novak, Pureblooded wizards.
Turns out Daneel was Tom Riddles younger sister, Daneel Riddle.  
Of course, the Riddles were related to Salzar Slytherin himself, and this made Hermione the living heir of Slytherin.  The problem with being a female Slytherin heir, is that they're cursed.  Once they reach a few months before the age of eighteen, they body starts to convulse, and change into what their mother looks like.  Another issue is that they must find a mate before their eighteen, or they'll die.  

Hermione keeps this secret between McGonagall and herself.

But then to make matters worse, she starts to grow feelings for Draco Malfoy, her enemy since her first year.  

What will happen when he is found out to be her mate?

Voldermort rose in Draco Malfoy's 3rd. At the end of the year. They did not know why or how or what happened. They had been attempting it, Draco knew despite his father's tight lips. Yet, the process had not gone as planned. It was the soul from the diary that had returned, not the 20-odd year old that they had been expecting. This came with...complications. Tom Riddle was 16. Ideally, he would go to hogwarts in order to recruit those that would remain loyal to only him. Sure he had the remnants that his older self had recruited, but Tom did not know them, and furthermore, he doubted the viability of many of them. So he uses them, but he wants more loyal followers, and as such he needs to go to Hogwarts. Quite a decent foundation for such things. Using the young Draco Malfoy, molding him, grooming him, he would fool Albus Dumbledore, gain access to Hogwarts, and then he would begin his recruitment process. If he found his mate, became a bit too attached (obsessed) , and started another feud, well all part of the process he supposed. There were other issues as well. With Remus Lupin, the werewolf, still teaching, and the favoritism amongst the students, they had some work to do, but it was nothing Tom couldn't handle.

Instead the remnant of soul still in the Diary of Tom Riddle with Lucius Malfoy once more rises instead and the deformed Voldermort dies right before Potter's eyes and escape leaving the Death Eaters in chaos and Potter believing Voldermort is dead along with Dumbledore. Totally confused, Lucius Malfoy and the Death Eaters now serve the 16 year old Tom Riddle, while at Hogwarts, the stupid Werewolf Lupin is teaching again to the annoyance of the Slytherins, Dumbledore and Lupin begins to suspect something is terribly wrong with Draco Malfoy when he appears a month into the school year with large bruises on his wrist and neck however, he refuses to say anything. Tom Riddle, is deep at work. He must infiltrate Hogwarts, but for that he needs trustworthy teen followers. Meaning those his own age, and Draco Malfoy is where he starts. Knowing that Draco is in desperate need of a friend, is easily manipulated as the boy has been abused by Lucius for several years, he orders Lucius to start bringing Draco to his childhood home, which is actually the home of Aver Jr, in an attempt to gain the boy's trust, unwavering loyalty, more than his father. He also wishes to trick Albus Dumbledore into believing that he's a harmless descendant of Tom Riddle/Lord Voldermort and get into hogwarts and start his plan to get revenge. Finally, when both he and Draco are in 6th year, he puts all he planned into action.
