Captured by two thugs, Albina Ratownik is held captive in an underground "hospital"that's buried twenty feet deep. She is in Dr. Smok's grip and is forced to undergo a painful psychological experiment. She is not the only victim here, there are many other "patients" that work as slaves, get their organs sold illegally, and are beaten in this so-called "hospital". The underground hospital is guarded by black-armored guards and is near impossible to escape. Not only does Dr. Smok torture his patients and watch them suffer but he takes pleasure in doing it. 

According to Dr. Smok,  Albina is special. She has a life that is similar to Smok's past which is why she was chosen for the psychological experiment. This experiment leave her alive with a mental scar or dead in the doctor's arms. Although escape is a death wish, Albina does find a chance of escape.
