Magic is real. It is terrible, blinding, agonizing and very, very real. When the use of your natural-borne talent is uncontrollable in the best circumstance and fatal in the worst, the only place to turn is to the Academy of Surgeons. For decades, they've been perfecting the modification of the human body to better adapt to magical ability and to lessen the pain brought on by using these talents. Because in this world, to fly is to feel a blade tearing into your back; to manipulate fire is to feel its' burn beneath your skin; to read minds is to drill a rusty bit into your skull and twist. Only the rich can afford to go to the Surgeons. And only the wildly talented ever become Surgeons themselves. The selective nature of the Academy - in staff, students, and clientele - means that any child born into poverty with the gift of magic is immediately in danger of being killed. Out of pity. While the upper class pay huge sums of money to have their bodies modified to fit their needs, the Surgeons quietly amass their fortune.
