A stranger wakes up on a table hooked up to a blood bag in an abandoned clinic, without knowing as much as his own name, and is thrust into the city of Yharnam. This city is cursed, it is plagued by an epidemic called the Scourge of the Beast, it turns the citizens of the city into terrible monsters that are just shells of their former selves.
The stranger must find out what is happening to this city, before it happens to him to.
