The date is the sixteenth of November, two thousand and sixteen, and the time is currently forty five minutes past one in the morning. One of our patients has had an extreme reaction to one of our solutions and is currently in recovery as we speak. I am currently with the patient to record notes for their file. Please introduce yourself for the tape. "......Subject 309....." Patient has been with us for a month, and has had no reaction since the first dose was administered. Doctors believe there was a slight chemical change in the dose which has lead to the patient having both their legs broken in several places including a mild spinal injury. No witnesses were present when this episode occurred. Patient refuses to cooperate and give more information on what occurred. Another interview will be attempted later in the day.
