Gotham is a hard place to live, let alone have a kid. And Harleen Quinzel knows better than anyone. Often times she's begged Joker for a child, and he said no all of those times except for one. The baby miscarried.
After five tries, Joker got fed up. Even went to Arkham to get away from Harley's tears and begging. So she adopts. With no Joker around to mess with her head anymore, Harley can only hope she can raise her new daughter to do different than she had.
