When Meredith and Derek are transfered to mercy hospital in New York they cross paths with Olivia and Elliot Stabler they never realised it would change their lives forever.
In a tragic accident Emily and Eli Stabler lose both their parents. Derek being the one who operated on them both feels a burning guilt that he can't seem to overcome.

What happens when both he and Meredith get custody of the Stabler children? Was this "accident" truly so accidental?
