Sakura doesn't know how she became anorexic, but Sasuke does.
He thinks it started with a low self-esteem and many insecurities, but slowly morphed into the need to feel like she had some control over her chaotic life. And thus began Sakura's tumultuous relationship with food.
As she slowly fell apart, Sasuke and her friends watched her become a shell of who she once was. Sasuke unknowingly develops a need to save her, but doesn't realize that she needs to save herself, first.
