All the fairy tale characters were transported to  Storybrooke with no memory of there past life's. Emma is in a mental institution because she thinks that everyone one is a fairy tale character. Killian is her nurse, he gives medicine, talks with her, cares for her. But nothing can stop true love, not even a Dark Curse.
