Dean had been going to therapy sessions ever since his brother Sam saw bloodied razor blades in the bathroom trash can. He was told by his therapist that whenever he felt like picking up that blade again, that he should write in his journal. Ever since then, he would make an entry -- But of course, he would remind the reader (which he assumed would be only himself) that it was NOT a diary -- talking about his week or even day. When he meets Castiel, things change for both the better and the worse...
