The list comes out every school year. Four pages long, one per class year, giving the best and worst titles to a select few students. Roxy Prefontaine is an average sophomore, no more noticeable than a fly on the wall. Of course, there was the rumor going around that she was a boy, but it wasn't a big deal, especially when bad boy Ricky O'Rourke takes a liking to her. And then Roxy's best friend starts dating Cait Henry, a quiet, nondescript girl they had never known before. The list puts Roxy and Cait at odds, and Oliver has to choose between his first girlfriend and his best friend. Meanwhile, Roxy's mother, Anna, tells her she's pregnant with her fiance, Jason's, baby and things start to heat up between Roxy and Ricky.

When something happens to Roxy, her whole world shifts upside down and everything she once trusted comes crashing down, who will she turn to? Ricky? Oliver? Her mother? Father? Step father? And how will her decision affect everyone else?
