7 year old Alice suffers from a disease that not even the doctors can figure out, no medicine they give her has worked. While in the hospital, her parents get her a 'get-well' present, a vintage jack in the box, but after she receives the present, she has been having nightmares that she can't explain and the clown is always missing. When the doors in her hospital room open in the middle of the night, everything makes sense of what happens at night.
