Castiel is captured and tortured by demons in search of Heavens secrets. What is the worst thing possible that they can inflict on a Holy Entity? It would take some thought. But apparently, as unfortunate as the situation could get and as Castiel returns to consciousness, he realizes that demons have a creative side.
