When Steve wakes up he realizes that the whole world has moved on without him, including his soulmate. Howard now has a son, Anthony. And now as he becomes Anthony's second father, he must live everyday seeing the name of his dead best friend on the body of his son, in bright red letters.
(Or, for a much less poetic and meaningful description, this is a Soulmate AU in which Steve and Howard are Soulmates, and so are Tony and Bucky, watch- or read, as Tony grows up and finally meets his mate, realizing that he's a lot more scarred than Tony could ever imagine. Also Tony lost his leg.)
