Dolph has alot of inner demons. Depression, anxiety, self harm.
During a match part of his wrist tape peels away and Dean sees the fresh scars. That's the first thing that tells Dean something is wrong.
He starts to notice things he hadn't before. Like the way Dolph's normally bright blue eyes were dulled. The way he almost never smiled or laughed anymore. and then there's the scars, and Dean wonders how long this has all been going on.
