The current time, Sherlock isn't blood related to Keto but lovers. One Above All died in a war against the 52's (current time) and somehow his soul went to Keto (current time). It screwed everything up. Now, the 52's from the past and 52's from the present time with Sharlotte who is creator god's original form must now go against the One Above All.
