Things have changed. 

For one, Klaus's only plan for the future is finding different ways to torture Marcel- and it's ruining him inside. He's becoming obsessive, and losing track of what really matters to him. 

Now with Caroline returning to Mystic Falls, he doesn't know how to feel. It's been years since he's seen the one girl he's ever truly loved. He was completely horrendous when she left for college. "An utter mess," as Rebekah described it.
