Okay so imagine Black Widow had a daughter with Bruce Banner, then that daughter had a kid with Captain America. That child would be the basics of Sam. He's terrified of his feelings and to talk to this girl. She makes him feel different and he doesn't know if he likes that. He still can't forgive himself for what he's done during his Winter Solider days. He sees Steve with her and realizes he might be too late and that she might not ever get to be his.
