Shinsou had taken a liking to Kaminari. Kaminari was known as Class 1-A's Pikachu or idiot. Shinsou had been dating Monoma though which he didn't really like anymore. Shinsou broke up with Monoma and confessed to Kaminari to find out their feelings were mutual. Monoma never knew when to give up so while Kaminari and Shinsou were dating Monoma tried to think of ways he could get them to break up. In Monoma's logic if they were to break up Shinsou would go back to him.

Kaminari has the Hanahaki Disease because he thinks Shinso doesn't love him. Shinso has the Hanahaki Disease because he thinks he isn't good enough for Kaminari's love.
