Izuku ends up punted back in time after Eri's quirk malfunctions and Aizawa is unable to erase it due to him being knocked the hell out. Izuku is back in his middle school days, more specifically the start of middle school, when he was about 13, though due to the unknown nature of Eris quirk and one for all, Izuku keeps a lot of his scars, his torn up Hero suit that is know a few sizes bigger than him, his phone, and his Memories of the future. (These will all come into play later)
One For All will end up back with All Might, since Izuku ended up back in his younger body the quirk disappeared.
