Draken and her are both twins because they were both born in the red light district. The people there abused both of them on daily basis and when y/n was forced to give service to an old man Draken felt enraged and took his sister and ran away from there before his sister could be more traumatized.
As of present they live in a brothel and the people there are nice who wouldn't abuse them or force y/n to give service. The older girls there protect and cherish them.
Draken, due to past incidents of his "friends" crushing on his twin(before he met mikey or mitsuya or anyone) he doesn't mention the fact that he has a twin sister and keeps her away from specifically gang members and gangs in general. The trauma of when y/n was abused and told to give service to a customer really haunts him. He sees her as a little girl who needs his constant protection but won't show it.
