The white rose's are all the people whoever existed on the Dream SMP, the red ones with Tommy's blood are the people who are close to Tommy like Tubbo and Ranboo.
This is resembling the blood he has lost for the people around him, being a lot. Tommy has given up his lives, disc's, his country, mainly everything he has ever enjoyed or gotten close to was taken from him. Thus leaving him with suffering and pain, depression and fear, hopelessness and lost.
But even though it hurt so much everytime anything has been taken from him, he still wants to give everything for those close to him, even if it meant killing himself slowly on the inside and outside.
