Death is the one that decide how.
Death is not one person, It's a group. Every living person know at least one death, just don't know it's a death.
Death all are controlled by the same person for a yaer, then another person, and another. The people are selected randomly and the way they control is by the thoughts.
The thoughts of the person in control is the way to decide:
How is that person going to die?
If the deaths loved that person, the person will die the way they wanted, if they didn't thought of that? Simple, their death will be sudden and unpainful.
If the deaths didn't like the person, they will die in the most crule way the person in control of the deaths think there is.
