Saiki can't see what people look like on the outside. This doesn't bother him. It's what's on the inside that really matters.
Saiko has an illness that could kill him if not treated. Saiki is taking extra care of him and hinting at the signs of sickness.
Saiko was being stubborn/didn't want to worry people so Saiki broke and hauled his ass to the hospital with everyone in tow because it was weird for Saiki to act this way.
