A few decades ago, One For All, the Pinnacle of quirks, belonged to Nana Shimura. She had two children, Kotaro Shimura and Inko Shimura.
Inko Shimura later went to UA alongside her mother's successor Toshinori Yagi. After school they kept their identities secret and continued a double life that only other family knew of, namely, Kotaro and his Wife.
Because of Nana's abandonment of her kids Kotaro began to hate Heroes and with Inko deciding to become one they drifted apart.
Kotaro had his own family that plays out just like canon.
Toshinori and Inko have a family of their own, with the couple having 4 kids.
Kotaro became an abusive father, and that led to Tenko Awakening his quirk when his father beat his mother and sister. He killed his father, and as his sister and mom comforted him, they also died.
Shortly after that incident, Tenko Shimura was adopted into Inko's family.
Inko had adopted the new surname of Midoriya as Yagi became public as All Might's name, this translated to all of her biological children also gaining that name.
