I was gonna make this first come first serve but since more people were interested than I though I might as well make it best form wins.  Like most people, I don't particularly care for aesthetics as long as content is there but I left the form kinda blank so you could do your own thing.  But I'm gonna say I don't just look at the form when I'm deciding which one to pick but Activeness.  I ain't gonna accept your form for you to just disappear.
