Trelawney isn't counting Peter Pettigrew, in disguise as Wormtail. Pettigrew makes 13, which means they were already at the critical number. And who was the first to rise?

Dumbledore, of course. Later in The half-blood prince Dumbledore dies, which means Trelawny was right about the the number 13.
