Olivia - Killed offscreen by the ghost of someone she couldn't save, as Lilith raised ghosts like these (as "the Witnesses") and turned them evil as part of breaking one of the seals. He was shown strangling her, but when her body was seen, there was a lot of blood.
Jed - Chest ripped out offscreen by one of the Witnesses (spirits of people hunters couldn't save).
Carl Bates - Is mentioned to have been viciously killed by one of the Witnesses.
J.C. Adams - Is mentioned to have been viciously killed by one of the Witnesses.
Witnesses - Went back to rest when Bobby performed a ritual, ending with Dean throwing a bowl into the fire. Among them are Ronald Reznick, Henriksen, Meg Masters, and two girls.
Tom - In the past, years before the first episode, was killed by Azazel in a deal made by his son, in exchange for permission to bleed into his kid's mouth in ten years.
Samuel Campbell - Set years before the first episode, was possessed by Azazel, who stabbed himself, killing Samuel but not really affecting Azazel at all. Was later resurrected by Crowley.

Luke Wallace - Killed by Don and Tracy with a hex bag, causing razor blades to appear in his throat. This was part of a sacrifice to break one of the 66 seals, and Don and Tracy are witches who work for Lilith.
Jenny - Drowned by Don and Tracy in the bobbing apple tub using a hex bag, as part of a sacrifice to break one of the 66 seals.
Don Harding - Shot in the back by Sam. He was intending to use Tracy as the final sacrifice to awaken Samhain and break one of the 66 seals, but with this Tracy used him as the final sacrifice instead.
Tracy Davis - Neck snapped by Samhain.
Justin - Dragged into the crypt and killed by Samhain. Injuries were not shown but blood was.
Sam Winchester - Struck by lightning because Hope wished for it with the wishing well. When Wes removed the magic coin and broke the spell, the wishes were all reversed and Sam came back to life.

Jeb Dexter - Hung on a noose from a ceiling fan, which was transferred from Jay's Executioner magic trick (which would have hanged him) to Jeb by Charlie with a tarot card.
Charlie - Killed when Jay stabbed himself, leaving a tarot card with Charlie to transfer the stab wound to him.
Taylor - Drowned in a toilet by April, who was possessed by the spirit of Dirk.
Boy - Hand shoved into a running food processor by another boy, possessed by Dirk's spirit. It seems the victim was a bully and the possessed one was a nerd.
Barry Cook - Is said to have committed suicide by slitting his wrists, long before the events of the first episode.
Dirk McGregor - Before events of the first episode, is said to have taken too much drugs, resulting in his death.
Dirk McGregor spirit - Destroyed when Dean burned a lock of his hair, which was all that remained of him.
Vicki - Killed by Adam with a meat tenderizer. He was under the Siren's influence. (Note: This is not the same Adam that is Sam and Dean's brother)
Mrs. Bristol - Killed by her son, Lenny, offscreen, under the Siren's influence. Though offscreen, the sounds of it could be heard.
Nick Monroe, the Siren - Stabbed by Bobby with a bronze knife dipped in Dean's blood (as Dean was infected by the Siren, which could only be killed by a bronze knife dipped in the blood of one of its victims).
Cole Griffith - Is said to have died of an asthma attack due to the cold. Returned as a spirit, though not an angry one, and later went on to the next world.
Reaper - Neck slashed by Alastair with Death's scythe as a sacrifice to break another seal. This seal was saved, though, as he did not get to kill Tessa.
Pamela Barnes - Stabbed by a demon.
Angel - Stabbed offscreen by Uriel, but died onscreen. She was not the first; In earlier episodes it is said that several angels have been killed.
Alastair - Killed by Sam using his power, which was now great enough to kill demons.
Uriel - Stabbed by Anna with an angel-killing sword.
Paul - Put his head in a microwave and turned it on (sticking a plastic fork in the latch of the door so the microwave would think the door was closed). His head exploded. He did this under the influence of Sandover's spirit.
Ian - Under the influence of Sandover's spirit, he stabbed himself in the neck with a pencil.
Guard - Decapitated when the elevator jerked down, with his head leaning into the elevator door.
P.T. Sandover spirit - Was destroyed when Sam burned his gloves with a lighter.
Kate Milligan - Killed offscreen by a ghoul. Her body is found.
Joe Barton - Killed offscreen by a ghoul. His body is found.
Adam Milligan - Killed offscreen by a ghoul sometime before the events of the episode in which the ghoul impersonating him appeared. His body is found, and he is later resurrected by angels on Zachariah's orders.
Kate Ghoul - Shot in the head by Dean.
Adam Ghoul - Head beaten in by Dean with a crowbar.
Roger - Possessed by a demon. While possessed his head was smashed in by Jimmy and his throat was slit by Dean.
