Alex was in a research facility where she met Stitches or her "innerself". Alex was alone in a dark room and was desperately trying to fight Stitches. When the men who took care of her first saw her they thought she was insane and told the scientist which took more tests on her mental state and health. The testing was causing her mental state to decrease and cause more Stitches attacks. They now lock her in a room alone with her hell.
