One malfunction. On tiny little thing that goes wrong, that's all it takes for something big to happen. And it did. Within minutes, the engine had totally failed, and then the rest went. All Katie could remember was that the Captain had started to announce something when the plane started falling. She had clutched her mum, and screamed with the rest of the passengers. She had been the only survivor, she realised when she swam from the wreckage. The only one to escape. One malfunction and her life as she knew it had been ripped from her forever.
