Sir's skull is a curse to him.
Because when he puts it on, he turns into his feral form and loses all control of his humanity.
When he doesn't wear the skull, voices in his head start up and chant to put it on. It drives him up the wall.

Sir's, or Thomas's feral form is twelve feet tall, and his human form is seven.
