So here's my example and YES I'm welcoming RFR.
You can just copy the main part the form, the extra stuff is just my personal pref in the request. This is just to give you an idea.
Here are your options. Choose between my stories, if you choose two stories to comment on sincerely, I will provide the same with two of your stories. The more you give me, the more I GIVE you :). (Make sure to read the rest of the tags, so you are made aware of what the story will consist of, in case it's not your thing.)
If one of my stories has more then 3 chapters, you are not obligated to read it all, however :) if you do, it would make me smile. I only expect you to read 3 and comment on the 3. They are stories I would prefer the read-to-read exchange, so I will put " PREF" meaning I really prefer you to read that one, but you don't have to, you can still pick the one you want.
