Before you fill in the form, go through the rules:

1) Your book should be published. Copyright and author's note is not included.

2) Request if you are 100% sure you are gonna use it. It takes a lot of hard work and time to make it.

3) Please be patient. Your covers will be ready within 1 week. If there is any delay then I will PM you.

4) You have to keep the cover for at least 2 weeks.

5) I will only accept the form once you do the payment. If I have any clarifications, I will PM you.

6) I have the right to deny your request.

7) I will only make changes in the cover two times. (Mistakes are not included)

8) Please fill in the form correctly.

9) No anime allowed.

10) It is up to you whether to give a shout out for the covers in your book but no taking credits.
